The aim of assault documentation is to ensure that the picture of the injuries can be understood precisely by independent third parties as well.

Photographs of the injuries, ideally taken perpendicular to the injury and with a scale, significantly facilitate understanding.

Written injury documentation should take into account the following points in particular:

- Precise details of the location of the injury

- A neutral description of the injury (shape, colour, delineation, size)

- Specification of the exact number of injuries

The use of body diagrams is ideal for clear documentation, especially of complex injury pictures.
